APPLIED OBSTETRICAL CARE

At the end of the course students will be able to:
1. Describe the reasons for the execution of the hand washing, the types of hand washing and the related procedures of enforcement.
2. Describe the normal values of vital signs and the factors that can influence them. Describe the procedures used to establish the body temperature, pulse, respiration and blood pressure and detection sites. Identify the equipment used for the assessment of vital signs.
3. Describe the reasons to perform the hygiene of the body. Describe the procedures of patient care to the bathroom or shower, Total hygiene, perineal hygiene, foot bath, oral hygiene, Eye and hair hygiene. Describe how to take off and put on clothing.
4. Describe the characteristics of diuresis and urine and the influencing factors. Describe the procedure for making extemporaneous catheterization and placement of indwelling catheter and the necessary material. Describe how to empty or replace the urine bag and the procedure for the removal of the bladder catheter.
5. Describe the characteristics of intestinal elimination and of faces and the influencing factors. Describe the procedure for the placement of a rectal probe and for the execution of an enema.
6. Describe the operator''''s responsibility in the execution of diagnostic tests. Describe the key diagnostic tests on urine, identifying the normal values. Describe the implementation procedures of the stick urine, physical-chemical examination of the urine, urine examination on a 24 hour sample, the collection of a urine sample midstream and collecting a sample of urine from a indwelling catheter. Describe the main diagnostic examinations of the feces, identifying the normal values. Describe procedures of the chemical physical examination of feces, the coproculture and examination for the detection of occult blood in the feces. Describe the key diagnostic tests on blood, identifying the normal values. Describe the procedures for capillary blood, the venipuncture, the arterial blood collection and blood culture. Describe the main instrumental diagnostic tests.
7. Describe the characters and the main purposes of the peripheral venous catheter. Describe the procedure for placement of peripheral venous catheter and identify any complications.
8. Describe the functions of the digestive system, and the main nutrients. Describe the parameters for the evaluation of the patient''''s nutritional status and major alterations of the same. Describe the hydration assessment of the patient by the execution of the hydroelectric balance. Describe the procedure on how to help to feed the patient.

o Hand hygiene:
Reasons for the execution of handwashing and fields of application.
Handwashing types: washing with detergent, washing with antiseptic and alcohol friction, surgical scrub.
o Vital signs:
Body temperature:
• physiology of thermoregulation.
• normal values and changes in temperature, causes of the temperature alterations.
• area and methods for the temperature detection.
The arterial pulse:
• physiology of the cardiovascular system.
• characteristics of pulse, normal values and alterations, causes of pulse disorders.
• area and methods of pulse detection.
The blood pressure:
• physiology of cardiac contraction and cardiovascular function.
• normal blood pressure and alterations, causes of pressure changes.
• area and methods of pressure detection.
Breathing:
• physiology of respiration.
• characteristics of breath, normal values and alterations, causes of breathing disorders.
• area and methods of breath detection.
o Hygiene of the person:
Skin functions and characteristics.
Principles to perform the hygiene of the body.
Teach the principles on how to perform the hygiene of the body.
Assisting while bathing or showering.
Total hygiene and perineal hygiene.
Footbath, oral hygiene, hygiene of the eyes and hair hygiene.
How to take off and put on clothing.
o Urinary excretion:
urinary tract anatomy and physiology.
Features of diuresis and urine and influencing factors.
How to promote urination and aids to ensure the urinary elimination.
Bladder catheterization: procedure for performing extemporaneous catheterization or indwelling catheter; drain and replace the urine collecting bag; procedure to remove the bladder catheter.
o Intestinal Elimination:
Gastrointestinal Anatomy and physiology.
Features of the feces, factors which influence feces elimination or alteration of it.
Insertion of a rectal probe and execution of a enema.
o Diagnostic and laboratory tests:
Operator Responsibility.
Urine test:
• reagent strips (Stick urine);
• physical and Chemical urine exam, pregnancy test;
• examination of a 24h urine sample;
• urine midstream (urincoltura);
• how to collect a urine sample from an indwelling catheter.
Feces test:
• chemical and physical examination of feces;
• coproculture;
• searching for occult blood in the feces.
Investigations on the blood:
• collection of capillary blood;
• venipuncture and types of blood tests;
• collection of arterial blood;
• blood culture.
Vaginal swabs.
Instrumental diagnostic Tests:
• non-invasive techniques of investigation; hints on radiography, scintigraphy, ultrasound, mammography, CT, MRI;
• invasive techniques; hints on colonoscopy, gastroscopy, angiography;
• suction procedures; hints on biopsies, thoracocentesis, paracentesis, lumbar puncture.
• procedures that assess the electrical conduction; hints of EEG, EKG and enforcement procedure.
